DESCRIPTION OF BENEFIT

• $5,000 term life insurance for Primary Member only. There is no coverage under this insured benefit for Legal Dependents.
   

• Eligibility and Effective Dates: This insured benefit is effective for the Primary Member on the date your completed application for Membership is received and processed.
   

• Exception: If you are confined in a Hospital or other health care facility on the date that coverage under this insured benefit would otherwise become effective; the effective date of coverage for this insured benefit will be deferred until the date you are released from such Hospital or other health care facility.
