Accidental Death and Dismemberment Benefit
Underwritten by U.S. Fire Insurance Company and/or Fairmont Premier Insurance Company.

  • This will provide up to $10,000 in Accidental Death and Dismemberment benefit for Elite, Platinum and Platinum Plus Members and up to $6,000 for the spouses and $5,000 for the dependent children.

DESCRIPTION OF BENEFIT

Eligible participants will be protected 24 hours a day, 365 days a year, for covered accidents occurring anywhere in the world, on or off the job, at home or while traveling. These program benefits are paid in a lump sum.

Accidental Death & Dismemberment Benefit: This plan will insure covered persons for the losses shown below that occur within one year from the date of a covered accident. The Principal Sum for this plan is $10,000 for members, $6,000 for spouses and $5,000 for each dependent child. If a covered person sustains more than one loss as the result of one accident, only one amount, the largest, will be paid.

Eligibility and Effective Dates: The insured benefit is effective on the first day of the month following the date your completed application for Membership is received and processed. Coverage for Legal Dependents you acquire after your coverage is effective will be effective on the first day of the month following the date you acquire the Legal Dependent.

Exclusions and Limitations and Restrictions: Benefits will not be paid for a covered person's loss which: (1) is caused by or results from the covered person's own: intentional self-inflicted injury, suicide or any attempt thereat; voluntary self-administration of any drug or chemical substance not prescribed by, and taken according to the directions of a doctor (Accidental ingestion of a poisonous substance is not excluded); commission or attempt to commit a felony; participation in a riot or insurrection; driving under the influence of a controlled substance unless administered on the advice of a doctor; or driving while intoxicated as determined by the laws in the jurisdiction of the geographical area where the loss occurs; (2) is caused by or results from: the covered person being intoxicated as determined by the laws in the jurisdiction of the geographical area where the loss occurs); Any narcotic, drug, poison, gas or fumes voluntarily taken, administered, absorbed or inhaled, unless prescribed by a doctor; Declared or undeclared war or act of war; injury sustained while in the service of the armed forces of any country; aviation, except as specifically provided in this Policy; Sickness, disease, bodily or mental infirmity or medical or surgical treatment thereof, bacterial or viral infection, regardless of how contracted. This does not include bacterial infection that is the natural and foreseeable result of an accidental external bodily injury or accidental food poisoning.

Aircraft Limitations: No benefits will be paid if the accident happens on a leased aircraft, an operated or controlled aircraft, or an owned aircraft.

Aircraft Restrictions: If the accident happens while riding in, or getting on or off, an Aircraft, benefits will be paid, but only if: (1) riding as a passenger in a commercial airline, and not as a pilot or member of the crew; and (2) the aircraft is not being used for: crop dusting, spraying, or seeding, fire firefighting, sky writing, sky diving or hang gliding, pipeline or power line inspection, aerial photography or exploration, racing, endurance tests, stunt or acrobatic flying or any operation which requires a special permit from the FAA, even if it is granted (this does not apply if the permit is required only because of the territory flown over or landed on).
